Proposal Outline and Requirements
- Project Title: Create an appropriate title for future reference to the project.
- Project Lead: Provide the name(s) and contact information (address, phone, fax, email) of the grant administrator and staff member who will serve as principal contact(s).
- Funding Request: Indicate the amount of funds sought and the amount provided 50-50 match.
- Abstract: Provide a brief summary of the project explaining its benefits to the community and/or neighborhood.
- Project Location: Proposals should identify the site and why it was selected. Define the status of site ownership and any required permits and municipal endorsements necessary to conduct the project. These endorsements/permits/permission letters may be included as an attachment.
- Methodology: Provide a brief description, preferably in a bulleted list, describing how the project shall be implemented showing approximate dates of completion of each step.
- Maintenance: Provide a two-year maintenance and watering plan specifying who the responsible parties are, and how the plan shall be implemented.
- Targets/Measures: Describe specific measures and targets that will be used to determine project success. All projects must be accompanied by an end-of-project evaluation identifying the targets/measures and if they were achieved. Failure to submit a final report may disqualify the applicant from future grant opportunities under this program.
- Partners: List/describe partners who will be involved and their level of commitment. Projects with partnerships between multiple groups or agencies, including UFA and the US Forest Service, are encouraged.
- Budget: Provide a proposed budget. Show primary budgeted activities and the source of all matching funds. Equal matching funds from non-federal sources are required and may include in-kind contributions of partners in the areas of staff time, work performed, materials and services donated (see attached budget form).
